OBJECTIVE: To evaluate the touch perception threshold of the alveolar mucosa and quality of life of edentulous patients before (T0) and 30 days after (T1) the insertion of new complete dentures. BACKGROUND: Touch perception is important so that edentulous patients can detect the position of complete dentures in their mouth. MATERIALS AND METHODS: This study included 32 complete denture wearers with good oral and systemic health and no temporomandibular disorders for at least 5 years. At each time point (T0 and T1), two tests were performed (touch perception threshold and quality-of-life scale). The touch perception threshold (Von Frey or Semmes-Weinstein test) was assessed using nylon monofilaments on the regions of the alveolar mucosa of the maxilla and mandible. The Oral Health Impact Profile for edentulous individuals (OHIP-EDENT) was administered to evaluate oral health-related quality of life. RESULTS: In the maxilla and mandible, the touch perception thresholds for all regions of the alveolar mucosa were significantly lower at T1 than at T0. The OHIP-EDENT mean scores showed that the overall quality of life was significantly better at T1 (3.6) than at T0 (4.7), and a significant improvement in quality of life was observed in all domains of the OHIP-EDENT at T1. For touch perception threshold, effect sizes ranged from 0.4 to 0.8; and for quality of life, effect sizes ranged from 0.4 to 1.0. CONCLUSIONS: New complete dentures reduced the touch perception threshold of the alveolar mucosa of both edentulous arches and improved individuals' quality of life.

OBJECTIVES: To compare the impact of three complete denture palatal rugae designs on participants' satisfaction and oral-health-related quality of life. BACKGROUND: Complete dentures palatal rugea are usually polished to a smooth finish, which can affect the patient's adaptation. Roughening or keeping an opening in the rugae area to replicate the natural sensation of the palatal rugae has been suggested but lacks clinical evidence. METHODS: This randomised crossover trial included participants with complete dentures randomly allocated to six sequences. Each sequence alternated between polished, roughened, and open rugae designs. Participants evaluated general and domain-specific satisfaction (eating, taste, speaking, phonetics, and ease of cleaning) on a 100-mm visual analogue scale. They also completed the Oral Health Impact Profile for Edentulous Patients (OHIP-EDENT) and indicated their preferred design at the end. Repeated-measure ANOVA compared satisfaction, and one-way ANOVA with Tukey's test compared OHIP-EDENT scores. RESULTS: Forty-two participants were randomised. Seven dropped out. General satisfaction ratings were comparable for the polished (Mean = 80.2, SD = 19.7) and roughened designs (Mean = 79.5, SD = 20.2). However, the open design had a significantly lower satisfaction rating (Mean = 41.1, SD = 32.2). OHIP-EDENT scores for the open design (Mean = 44.9, SD = 17.5) were significantly worse than those for the polished (Mean = 36.1, SD = 12.7) and roughened designs (Mean = 36.2, SD = 13.3). Two-thirds of the participants chose the polished design. One-third chose the roughened design; of those, 83% were first-time denture wearers. CONCLUSIONS: Complete dentures with polished and roughened rugae designs were perceived similarly. Both designs could be recommended based on the patient's preference. However, the open design should be avoided due to negative perception.

This study is the first study to incorporate a novel approach to single-implant-retained mandibular overdenture with a bar attachment on a single implant in the symphysis region. The use of a single-implant-retained bar overdenture significantly improved patients' chewing efficiency, indicating enhanced overdenture retention and stability. Materials and Methods: Ten maxillary and mandibular completely edentulous patients were enrolled in the study. A complete denture was fabricated for them. On completion, bite force, retention force, and bone change were recorded. Bite force was recorded with the strain gauge, retention force with the force measurement gauge, and bone changes with radiographs. Conclusion: Single-implant-supported bar mandibular overdenture (SISBOD) can be a suitable standard economic option for edentulous mandible and can greatly improve the quality of life of patients.

Aim: To identify the optimal number and position of implants to reduce stress concentration on the implant, denture, and attachment system for sustaining an overdenture prosthesis. Materials and methods: By incorporating one to eight indigenous implants with bar-type attachments, eight 3D finite element models of mandibular overdentures were created. All models received a 200 N vertical load, and the biomechanical characteristics of peri-implant bone were assessed. Result: The study observed that with a vertical load of 200 N, the maximum equivalent stress around peri-implant tissue in all models was within the physiological tolerance threshold of bone. The von Mises stress values ranged from 116.18 MPa to 536.7 MPa. Conclusion: The three-implant-supported overdenture model revealed superior peri-implant stress, stability, cost-effectiveness, and hygiene maintenance outcomes. Placing a third implant in the mid-symphysis region may offer a practical solution to reduce rotations in two-implant-supported overdentures.

OBJECTIVES: This study aimed to compare the physical and mechanical properties of four denture base materials: Polyan IC (PA), milled polymethylmethacrylate (PMMA), three-dimensional (3D)-printed resin (3DP), and SR Ivocap (SR). METHODS: Ninety-six samples were prepared and divided into four groups as follows. Group A consisted of 3DP (Asiga DentaBASE, Asiga) fabricated using a manufacturer-recommended 3D printer (Asiga Pro 4k, Asiga). Group B comprised milled PMMA (MP) (Ivotion Base, Ivoclar Vivadent). Group C included PA (BredentSenden), meanwhile, group D involved SR (Ivoclar VivadentSchaan). Cuboid samples (65 mm x 10 mm x 2.5 mm) were used for biaxial flexure strength testing in a universal testing machine (UTM). Cylindrical samples of 20 mm x 40 mm were used for compressive strength testing in a UTM. Additionally, cuboid samples (65 mm x 10 mm x 2.5 mm) were used for Vickers surface hardness testing in a microhardness tester. Disc samples (10 mm x 2.5 mm) were employed for color stability testing both in a coffee solution and Coca-Cola, using a digital spectrophotometer. Statistical analyses were performed using one-way analysis of variance and Tukey's post hoc analysis (α=.05). RESULTS: MP demonstrated superior compressive strength (p=0.002) and color stability compared to that exhibited by 3DP (p<0.001) while displaying similar flexure strength (p=0.336) and hardness (p=0.708). MP and PA displayed similar compressive strength (p=0.081), flexure strength (p=0.159), and color stability in coke (p=0.071). However, MP had reduced hardness (p<0.001) and color stability in coffee (p<0.001). Moreover, MP demonstrated a higher compressive strength (p<0.001) than that displayed by SR. However, the flexure strength, hardness, and color stability were similar (p > 0.05). Furthermore, 3DP exhibited comparable compressive strength (p=0.334) to that of PA but demonstrated significantly lower flexure strength (p=0.005), hardness (p<0.001), and color stability (p<0.001) compared to PA. In comparison to SR, PA had a higher compressive strength (p <0.001), hardness (p=0.001), and color stability in coffee (p<0.001), although they demonstrated similar (p>0.05) flexure strength and color stability in coke. CONCLUSIONS: The MP and PA demonstrated superior compressive strength than that exhibited by the other materials tested. The tested materials had similar flexure strengths, except for PA which demonstrated superiority over the 3DP. Among all tested materials, PA exhibited the highest hardness, while the 3DP was the least color-stable. CLINICAL SIGNIFICANCE: Considering the mechanical properties and color stability, Polyan and milled polymethylmethacrylate are preferred for complete denture fabrication. However, the limited repairability and complex handling of Polyan should be considered.

Complete denture is a real challenge for any practitioner. The aim of implantology is to optimize prosthetic balance and ensure that prostheses are perfectly integrated from a bio-functional viewpoint, despite an unfavorable anatomophysiological context. Here, we present a case managed in our department concerning a mandibular complete removable denture retained by a medial symphyseal implant. A 61-year-old fully edentulous mandibular patient with a Kennedy Applegate class I edentulous maxilla in good general health consulted for the renewal of his mandibular complete denture, which was deemed unstable and non-retentive. The exo-oral examination was normal. The mandibular crest was heavily resorbed, especially posteriorly, and covered with slightly inflammatory fibromucosa. The patient refused bone augmentation surgery. We performed a piezographic mandibular prosthesis retained by a single standard implant at the level of the mandibular symphysis. The prosthesis was stable and retentive. Masticatory comfort and efficiency were satisfactory and the psychological integration of the prosthesis was improved. Several studies have shown that a single symphyseal implant is a therapeutic alternative that completes the therapeutic range in specific cases. It should be limited to the mandibular arch in elderly patients with reduced bone volume. Given the lack of randomized controlled trials, routine use of this new approach is not recommended, and further studies are required.

AIM: The goal of this research was to facilitate dialogue and interaction among a group of dental practitioners about their views on denture adhesives, with the objective of reaching a collective consensus through the application of the Delphi Method. METHODS: This investigation employed the Delphi Technique, involving multiple rounds of questionnaires sent to a panel of experts. The objective was to establish a consensus (defined as over 70% agreement) or to explore the different viewpoints on the subject. A comprehensive Delphi questionnaire comprising 22 items was distributed to the Dental Practitioners. The questionnaire covered five key areas concerning denture adhesives: (1) overall perceptions; (2) the role in the development of clinical conditions; (3) specific applications and potential misuses; (4) their importance in denture services, including educating patients about denture adhesives; and (5) a general assessment of their clinical applications. RESULTS: All 31 panellists, chosen randomly, unanimously agreed to participate, with each of them actively involved in all three rounds of the survey. The panellists reached a consensus and definitively concluded that denture adhesives (1) are beneficial for enhancing the fit and comfort of the prosthesis and are not used to mask the underlying denture problems; (2) cause diseases such as denture stomatitis, candidiasis, and resorption of alveolar bone; (3) have the potential for increasing retention, function, and reducing patient anxiety. However, the panellists did not achieve a consensus on whether denture adhesives cause nausea and gagging in the patients; (4) education is very important for patients with both ill-fitting and well-fitting dentures; (5) are a beneficial adjunct to dentists when fabricating dentures. CONCLUSION: The expert panel of distinguished dental practitioners determined that denture adhesives play a valuable supportive role in the field of denture prosthetics, serving important functions during both the creation and post-insertion stages of dentures.

The absence of teeth, known as edentulism, poses considerable obstacles in prosthodontic care and greatly affects a person's well-being. Conventional complete dentures frequently lead to problems like instability and insufficient retention, especially in the lower jaw. Fortunately, the introduction of dental implants has transformed the way we approach edentulous patients, as they now offer support and enhanced retention for removable prostheses, thus revolutionizing their treatment. While a consensus exists on using two implants for retaining mandibular overdentures, the associated cost may be prohibitive for economically disadvantaged individuals. As a solution, the concept of single implant-retained mandibular overdentures has emerged, catering to individuals with limited financial resources and complete tooth loss. This review explores the efficacy and suitability of the single implant overdenture approach, along with an overview of treatment options for edentulous patients, including traditional dentures, tooth-supported overdentures, and implant-supported overdentures. The preservation of bone, improvements in functional abilities, and psychological benefits associated with overdentures are discussed. Moreover, various classifications and prosthetic options for implant overdentures, specifically for mandibular cases, are presented. This literature review aims to provide a comprehensive understanding of possible treatment options and focus on the single implant-retained mandibular overdenture approach and its implications in prosthodontic rehabilitation for edentulous patients.

PURPOSE: When carrying out prosthetic rehabilitation of edentulous and partially edentulous patients, great attention is paid to the personal attitude of the patients, their satisfaction with oral health and psychosocial interaction due to tooth loss, as well as the treatment of the resulting disorders. This attention has led to the development of various instruments for examining the quality of life related to oral health. The aim of this study was to develop and validate a reliable instrument in the Serbian language suitable for measuring oral health-related quality of life in patients who have been rehabilitated with complete or partial dentures. Мaterials and Methods: The study was unicentric and cross-sectional, and assessed the reliability and validity of a newly developed instrument for measuring the oral health-related quality of life in denture wearers (OHRQoL-DW). It was conducted on a sample of 200 adults from Serbia, wearers of various types of dentures, with a mean age 66.9 ± 10.3 years and male/female ratio of 86/114 (43%/57%). RESULTS: The definitive version of the OHRQoL-DW scale with 28 items showed very good reliability, with Cronbach's alpha = 0.938. Good temporal stability of the questionnaire was demonstrated, and satisfactory results were obtained for divergent and convergent validity tests. Exploratory factorial analysis revealed four domains of oral health-related quality of life in denture wearers: physical, psychosocial, environmental and aesthetic. CONCLUSIONS: The OHRQoL-DW scale is a reliable and valid generic instrument for measuring the oral health-related quality of life in patients wearing dentures, which is one of the most important outcomes of oral health in prosthetic treatment.

PURPOSE: This systematic review examined the effectiveness of soft denture relining (SDR) materials. STUDY SELECTION: A comprehensive search of MEDLINE, Cochrane Library, and ICHUSHI was conducted up to July 26, 2020. Target outcomes were patient satisfaction, oral health-related quality of life (OHRQOL), masticatory ability (MA), denture functional duration, residual ridge resorption (RRR), and microbial contamination. An organization specializing in literature searches performed the reference searches, and two reviewers independently selected the literature sources, extracted the data, and assessed the risk of bias. The reviewers resolved any disagreements concerning the assortment of literature sources through discussion. SDR included acrylic- and silicone-based materials, which were evaluated separately. RESULTS: Reviewers selected 7, 5, 11, 1, 4, and 6 studies to assess patient satisfaction, OHRQOL, MA, functional duration, RRR, and microbial contamination, respectively. The results confirmed that SDR improved patient satisfaction, OHRQOL, MA, and RRR. However, the functional duration of SDR material is shorter than that of hard denture relining (HDR) or acrylic resin material. Furthermore, SDR material is more susceptible to microbial contamination in the long term. The risk of bias for the included studies tended to be high because of specific issues (difficulty in blinding SDR versus HDR). CONCLUSIONS: For patients who wear complete dentures, SDR often provides beneficial outcomes such as pain reduction and recovery from MA. However, caution should be exercised regarding their use owing to insufficient functional duration and the possibility of microbial contamination during long-term use.

OBJECTIVE: To evaluate clinical parameters that affect the performance of root-supported overdentures (ROD) and to identify the possible factors that may affect it. BACKGROUND: ROD still serve as an alternative and successful treatment plan nowadays for partially edentulous patients. Nonetheless, there is not yet conclusive evidence about their clinical evaluation. MATERIALS AND METHODS: A search strategy was developed following a PIO (Population, Intervention, Outcome) framework including an electronic search in the following databases: PubMed, Cochrane Library and Scopus. The systematic search included only randomised controlled clinical trials (RCTs), published until January 2023 in English language and was performed by two independent reviewers. Quality assessment of the included studies was conducted according to the Cochrane Risk of Bias tool. RESULTS: The final selection of studies included 11 RCTs. Clinically relevant variables derived from this search were: Complications, with caries being the most frequently reported, periodontal aspects and bone changes around abutment teeth. Additionally, methods and clinical suggestions for the maintenance of these restorations and patient-related outcomes were assessed. According to the risk of bias assessment, nine studies were considered of high risk, whereas only 2 of low risk. A meta-analysis was not feasible. CONCLUSION: Within the limitations of this systematic review, ROD remain a viable treatment plan, offering better denture stability and bone preservation around abutment teeth and high patient satisfaction. Thorough oral hygiene and regular follow-ups are of utmost importance. Further well-designed RCTs are necessary for firmer conclusions.

OBJECTIVE: To present clinical strategies for prosthetic rehabilitation with complete dentures (CDs) of a Parkinson's disease patient. CASE REPORT: An 82-year-old patient sought the Department of Dentistry at UFRN, reporting retention dissatisfaction and hamper to the mandibular CD adaptation. Patient-reported a dry mouth sensation, and exhibited disordered mandibular movements, tremors, and resorbed mandibular ridge. Aiming retention and stability, double molding with zinc enolic oxide impression paste, neutral zone technique, and non-anatomic teeth were proposed as clinical strategies. At delivery, identification and relief in the supercompression areas were performed to facilitate the acceptance and use of the new dentures. CONCLUSION: The strategies promoted patient satisfaction regarding retention, stability, and comfort. This treatment may be considered for the rehabilitation of Parkinson's disease patients, favoring the adaptation process.

PURPOSE: The purpose of this study is to investigate the effects of denture adhesives on masticatory performance via a 10-center, parallel, randomized, controlled trial of complete denture wearers in Japan. METHODS: The trial was conducted between September 2013 and October 2016. The inclusion criteria were complete edentulism, willingness to undergo new complete denture treatment, and willingness to return for recall treatment. The exclusion criteria were age 90 years or older, presence of severe systemic illness, inability to understand the questionnaires, wearing metal base complete dentures, denture adhesive user, wearing prosthetics for maxillofacial defects, wearing complete dentures with tissue conditioners, and severe xerostomia. Randomization of the powder-type denture adhesive (powder), cream-type denture adhesive (cream), and control (saline) groups was performed using a sealed envelope system. Masticatory performance was measured using color-changeable chewing gum. Intervention blinding was not feasible. RESULTS: Sixty-seven control, 69 powder, and 64 cream participants are analyzed using the intention-to-treat principle. The participants in all groups show significantly improved masticatory performance at post-intervention (paired t-test with Bonferroni correction P < 0.0001). However, no significant difference in masticatory performance is detected among the three groups (one-way analysis of variance). A significant negative correlation between pre- and post-changes in masticatory performance and intraoral condition scores is observed (Pearson's correlation coefficient, P < 0.0001). CONCLUSIONS: Although denture adhesives improved the masticatory performance of complete denture wearers, their clinical effects are comparable to those of saline solution. The use of denture adhesives is more effective in complete denture wearers with unsatisfactory intraoral conditions.

INTRODUCTION: Advances in CAD-CAM complete dentures open up opportunities in preclinical laboratory education. A cross-over study was conducted to assess the benefit of digital training in custom tray fabrication. Hypotheses were that digital training improved conceptual skill acquisition and that assessment of digital work helps in the discrimination of students' difficulty. MATERIALS AND METHODS: Third-year students were allocated either into group A and took the manual practical classes before the digital ones, or into group B (N = 154). Prior to the sessions, a motor skill test was conducted. The influence on the learning process was evaluated by comparing the groups' results to the manual lab work with a Student's t-test. The effectiveness of the assessment in discriminating manual aptitude and conceptual skill was studied through Spearman's rank coefficient between digital and manual scores in conceptual skill and with a subgroup analysis according to the results of the motor skill test. The level of significance was set up at .05. Students' satisfaction was also assessed with a questionnaire. RESULTS: Students in group B achieved better scores. Spearman's rank coefficient test showed a weak correlation, R = .16. For manual lab work, the most manually skilled students did significantly better. In contrast, for the digital session, the medium manually skilled students did better. More than half of the students (55%) found the manual and digital sessions complementary, without identifying any difference depending on the learning sequence. CONCLUSION: Digital training improved the students' results in the conventional practical exercise. Assessments of the digital and conventional custom trays were complementary in order to distinguish conceptual and motor skills.

PURPOSE: To assess, clinically, patient satisfaction of additively manufactured complete dentures with intraoral scanning and hybrid cast digitization in comparison with conventional complete dentures. MATERIALS AND METHODS: Participants who were edentulous in both arches were recruited and received three types of complete dentures (CDs): conventionally manufactured with conventional impression (CC), additively manufactured with intraoral scanning (AMI), and additively manufactured with cast digitization (AMH). Definitive impressions of the edentulous arches were made with medium viscosity polyvinyl siloxane (Hydrorise Monophase; Zhermack, Italy) for the CC group, intraoral scanning (TRIOS 4; 3Shape, Copenhagen, Denmark) for the AMI group, and laboratory scanning of the definitive casts (Ceramill Map400 AMANNGIRRBACH, Pforzheim, Deutschland) for the AMH group. The trial dentures of the CC group were scanned for occlusion registrations of the AMI and AMH groups and were used to guide the designing process (Exocad 3.0 Galway; Exocad GmbH). The AMI and AMH dentures were additively manufactured with a vat-polymerization 3D printer (Sonic XL 4K; phrozen, Taiwan). Patient satisfaction and clinical outcome were assessed with OHIP EDENT, and 14-factor criteria, respectively. Statistical analyses were performed with paired sample t-test and one-way repeated measure ANOVA for satisfaction, Wilcoxon signed rank test for clinical outcome, and Pearson's r (r) for effect size, with α = 0.05. RESULTS: A total of 20 participants were included. Satisfaction had no statistically significant difference within or among the groups (p < 0.105). Within-group comparison between the two arches showed no statistical significance for the clinical outcome except for a significantly higher maxillary AMI score (p = 0.01, r = -0.40 with medium effect size). For among group's comparison; AMI had a significantly lower score than CC for the maxillary and mandibular arches (p = 0.01, r = -0.40, medium effect size, and p = 0.003, r = -0.47, medium effect size), and significantly lower score than the mandibular AMH (p = 0.03, r = -0.47, medium effect size), with significantly lower quality in teeth arrangement and retention domains for the AMI, and teeth arrangement for the AMH, in comparison with CC. CONCLUSIONS: Patient satisfaction with both types of additively manufactured dentures is comparable to conventional dentures. The comparable overall clinical outcomes between hybrid and conventional dentures indicate that additive manufacturing is an acceptable clinical substitute for the conventional methods. However, additively manufactured dentures made with intraoral scanning have lower clinical quality and retention than hybrid and conventional dentures, particularly for the mandibular arch. Teeth arrangement of both additively manufactured dentures is clinically inferior to the conventional denture.

During the fabrication of a complete denture, functional impression is taken. Literature studies show that polydimethylsiloxane (condensation silicone) has not been reported by United States dental schools to perform border molding. Thus, the purpose of this article is to review the functional impression technique when border molding is performed with a laboratory condensation silicone putty.

Statement of the Problem: The common causes of obstructive sleep apnea (OSA) are identified as anatomic and/or functional abnormality in the oral cavity, oropharynx, velopharynx, and hypopharynx leading to compromised airway space and increased collapsibility. Purpose: This study was conducted to evaluate the effect of implant-supported mandibular complete denture in improving the airway space among completely edentulous patients with OSA and compare it with conventional complete denture. Materials and Method: In this observational study, completely edentulous individuals were screened with snoring, tiredness, observed apnea, high blood pressure, body mass index, age, neck circumference, and gender (STOP-Bang) questionnaire to evaluate the incidence of OSA. Ten mild-moderate patients were included as study participants. Lateral cephalograms (L1) made at the edentulous state was considered baseline. They were rehabilitated with complete denture prosthesis. One week after denture insertion, two implants were placed in the edentulous mandibular arch. Delayed loading protocol was followed. Lateral cephalogram (L2) was made 6 months after complete denture insertion and 6 months after implant-supported prosthesis (L3). Cephalometric tracings were used to evaluate change in upper airway space (UAS), middle airway space (MAS), and lower airway space (LAS). Repeated measures ANOVA was used to evaluate statistical significance in the airway measurements made at the three intervals. Post hoc Tukey HSD and Bonferroni test were used to assess if the differences obtained were truly significant. Results: Statistical analysis revealed significant differences in UAS, MAS and LAS between L1, L2 and L3 (p< 0.05). Post hoc Tukey HSD indicated that UAS increased significantly at all three intervals followed by LAS and MAS respectively (α=.05). Post hoc Bon-ferroni test indicated that implant-supported mandibular complete dentures had a significant improvement in airway space when compared to conventional complete dentures (α=.05). Conclusion: Implant-supported mandibular complete denture could be effective in edentulous patients with mild-moderate OSA.

The aging population in developed countries has increased the number of edentulous patients and, therefore, the need for prosthetic rehabilitation to improve their quality of life. Complete dentures are the main treatment option in these cases. The use of CAD/CAM (Computer Aided Design/Computer Aided Manufacturing) in dentistry has improved clinical protocols and outcomes, achieving a reduction in work time and economic costs for the patients. The main objective of this review was to compare the characteristics of conventional and digital dentures, attempting to determine whether the use of new technologies represents an improvement in the properties of removable complete dentures. A bibliographic review was carried out in the PubMed/MEDLINE, Cochrane Library, Scielo, and Embase databases. With the initial search, 157 articles were obtained. After applying the inclusion and exclusion criteria, 64 publications were selected for this bibliographic review. The different conclusions of the studies consulted were compared regarding fit and retention, fracture resistance, surface roughness, biocompatibility, and aesthetics, taking into account the different methods of prostheses fabrication. In general, digital prostheses have shown better mechanical properties and, consequently, better biocompatibility and aesthetics than conventional prostheses. However, the obtained results were very heterogeneous, preventing a supported conclusion.

Aims: The aim of this study was to assess the impact of prosthodontic rehabilitation on glycemic and lipid control in functionally and completely edentulous patients with diabetes. Setting and Design: An in vivo study conducted with the intention of studying the potential link between edentulism and impaired masticatory efficiency with the nutritional status in diabetic patients. Materials and Methods: A total of 20 diabetic patients based on the inclusion criteria were selected. They were rehabilitated using a removable prosthesis, and observations were made across three parameters - glycosylated hemoglobin (HbA1C), serum cholesterol (S col), and serum protein (SP) at three stages - baseline, 3 months, and 6 months posttreatment. This was done to gauge the impact of the prosthetic rehabilitation on their health due to an increased masticatory efficiency potentially causing changes in dietary patterns. Statistical Analysis Used: •Inter group comparison (>2 groups) was done using one way ANOVA followed by pair wise comparison using post hoc test. •Intra group comparison was done using repeated measures ANOVA (for>2 observations) followed by post Hoc test. For all the statistical tests, P < 0.05 was considered to be statistically significant, keeping α error at 5% and ß error ati20%, thus giving a power to the study as 80%. Results: Hba1c at the baseline had a mean value of 8.04%, which reduced to 7.87% at the 3-month stage and 7.38% at the 6-month stage. S col at the baseline had a mean of 151.6 mg/dL; at the 3-month follow-up, it was 166.5 mg/dL, and at the 6-month follow-up, it was 173.95 mg/dL. SP had a mean baseline value of 6.38 mg/dL, which progressed to 6.67 mg/dL at the 3-month stage and 6.97 at the 6-month stage. Conclusion: Within the limitations of this study, it can be concluded that after 6 months of prosthetic rehabilitation in edentulous/functionally edentulous patients: There was a reduction in HbA1c (8.04%-7.38%); however, it was found to be statistically insignificant at that stage There was an increase in S col (151.6 mg/dL-173.95 mg/dL); it was found to be statistically significant There was an increase in SP (6.38 mg/dL-6.97 mg/dL); however, it was found to be statistically insignificant at that stage.

OBJECTIVES: The aim of the present study was to assess the long-term course of OHRQoL and the impact of the loading protocol in patients receiving a single mandibular implant supporting a complete denture over a period of five years. METHODS: In this multicenter RCT, a total of 158 edentulous patients were initially included and were randomly allocated immediately after placement of a mandibular midline implant to either immediate loading (IL) or to conventional loading (CL) with submerged healing. The assessment of OHRQoL was performed with the 49-item Oral Health Impact Profile (OHIP) at baseline and 1, 4, 12, 24, and 60 months after loading. At 5-year follow-up, 100 patients (mean age: 69.2 years; 45.0% female) with completed OHIP were available for analyses. A mixed-effects model with patients as random effect and an unstructured covariance matrix was developed to address repeated outcome measurement. RESULTS: The OHRQoL improved substantially after loading, indicated by a decrease of mean OHIP summary scores from 51.0 points at baseline, by 14.2 (95%-CI: 9.4 - 19.1; p<0.001) points to 37.2 points at 1-month follow-up, and by continuous improvement to 20.4 OHIP points at final follow-up. Considering constant treatment effects, the loading protocol had no significant effect on OHIP scores (-3.7, 95%-CI: -9.4 - 2.2; p = 0.204). Time effect was statistically significant with -0.21 (95%-CI: -0.28 - -0.15; p<0.001) points per month. CONCLUSION: Both the immediate and conventional loading of a single mandibular midline implant supporting a complete denture offer long-lasting high levels of OHRQoL, with no significant or clinically relevant long-term differences. CLINICAL SIGNIFICANCE: The study firstly presents long-term data for OHRQoL by investigating the loading protocol of single mandibular implant-supported complete dentures. Since immediate loading has been associated with a reduced implant survival rate for this concept, information on patient benefits is essential for evidence-based decision making.
